Make check or money order out to: "Absolute  BEST  Driving School" and a letter including your child's:

1) full legal name 2) birth date 3) high school 4) home address or mailing address

5) home phone number, child cell, and a parent cell 6) your email address for confirmation (optional)

7) and the service you are requesting, or whether your child as a permit or not.
